About this template
A result that leaves the laboratory carries a signature and a liability. Drafting method text, explaining an assay pattern or answering a clinician query all sit close to that boundary without crossing it. This starter is built to keep the crossing deliberate. Health identifiers are masked before the prompt leaves, and the answer must pass a numeric floor before it returns.
A laboratory director usually asks a narrow question. Staff want to draft a method note or explain a result pattern without turning that draft into a reported interpretation. The controls here separate the two. They govern the traffic and leave the signed result with the qualified professional.
Requests pass four gates before the model sees them. Jailbreak screening runs at the front, which keeps a planted instruction from reaching the access rule and weakening it. That access rule refuses any caller without verified identity. The health data stage layers laboratory relevant heuristics onto the same redaction path. Masking then hides patient numbers, member record numbers and payer policy numbers.
Scoring on the response side requires 0.88 in aggregate, and it weighs relevance and accuracy equally at half each. Relevance carries its own floor of 0.78 and accuracy carries one of 0.80. A draft shorter than 120 characters or built from one sentence fails before any score is read. The default outcome for a failure is refusal, not a softened answer.
Three boundaries deserve a note. Metadata only history means the text of a request never enters that record, which suits a laboratory under a no retention rule. The audit marker adds an allow only signal and does nothing to storage or retention. An oversight step has no rule inside it yet, so no answer routes to a reviewer until you write one. Source checking flags a weak citation and lets the answer through, which is the opposite of what many readers assume.
